Abstract

this study was conducted on 120 patients with brain metastasis presented to Radiation Oncology Unit Zagazig University Hospitals from February 2010to February 2011.
This was a comparative follow up study, so patients were allocated into four therapeutic groups:
Group A: Included 30 patients received 30 Gy over 10 days (300 cGy/fraction).
Group B: Included 30 patients received 20 Gy over 5 days (400 cGy/fraction).
Group C: Included 30 patients received 12 Gy over 2days (600 cGy/fraction).
Group D: Included 30 patients received 10Gy over 1day (1000 cGy/fraction).
In our study the median age is 47.5 years and the range is between 30-70 in group A , median age is 49 years and the range is between 30-73 years in group B, median age is 54 years and the range is between 30-70 years in group C and median age is 51 years and the range is between 30-76 years in group D.
The percentage of female patients is higher than that of males in both groups (53.3 % versus 46.7 %) and (60.0% versus 40.0 %) in group A and B respectively ,while alternaing percentage of female patients to male (46.7 % versus 53.3 %) in group C and D respectively.(D 53.33 VS 46.7).
The commonest symptom was headache which constituted (93.3%) in group A,(86.6%) in group B,D while 80% in group C.
In our study karnofsky performance status (KPS) ≤ 70% in all studied groups.
As expected the commonest primary sites of tumor were at lung and breast among studied groups respectively.
In our study higher percentage of patients have no extracranial metastastes than patients with extracranial metastastes in groups A, B and D( 66% )vs (33%) & (73%) vs (20%)&( 60%)vs (40%) respectively, while in group C there is higher percentage of patients have extracranial metastastes than patients with no extracranial metastastes (60%)vs (40%).
There was improvement of subjective response after treatment as the highest percentage of patients in group A and B have mild symptoms (67% vs 60%) respectively than group D and C (27% vs 13%) respectively although 4 patients(13%) of group C have no symptoms . However only 6.7% of cases in both groups A and C complain of severe symptoms rather than 13.3% in group B and D, no cases in group A,C and only two case (7 %) in group B while six cases(20%) in group D complains of extremely severe symptoms.
We observed quite similar results in the four fractionation groups for overall survival for 3Gy × 10 , 4Gy × 5, 6 GY× 2 and 10 GY× 1 groups.
All treatment groups were well balanced with respect to potential prognostic factors, improved survival in all groups but with no statistical significance.
