Abstract

Although the small bowel represents 90% of the surface area and 75% of the length of the alimentary tract and is located between two organs with high cancer incidence (i.e., stomach and colon), malignant neoplasm of the small bowel fall in the category of rare neoplasms. They account for only 2% of all GI malignancies.
The aim of this work was hospital based registration of the tumors of the small intestine (in Ain Shams University Hospitals in the period from 2001-2005), in order to assess the histopathological features and to detect the relative frequencies in relation to the total tumors.
The cases of the study were registered from the pathology labaratory of Ain Shams University Hospitals including El Demerdash and Ain Shams University Specialized Hospital in the period from 2001-2005.
Revision of the slides for histopathological evaluation was done , also the available clinical data as regards age, sex and presentation were collected from files of Ain Shams Hospitals during the period from 2001-2005.
The study included 37 cases; (89%) were malignant and (11%) were benign, from the malignant tumors of small intestine (33 cases), (87%) were primary tumors and (13%) were secondary tumors.
The results of the study showed that the total number of tumors of small intestine represented approximately 0.3% of all body tumors. Concerning the age of the patients, it ranged from 3 to 75 years except only one case aged 1 month (a case of well differentiated adenocarcinoma). The mean age was 46.3 years. The commonest age group was from 41-60 years representing 40.5%, followed by the age group from more than 60 years representing 29.7%. This registry comprised 21 male cases representing 56.7% and 16 female cases representing 43.2% with a male to female ratio about 1.3:1. Concerning the site of tumor, the duodenum was the most common site of tumors of small intestine representing 24.3%, followed by the jejunum representing 21.6% of all cases.
The results of the study showed that the total number of malignant tumors of small intestine represented approximately 0.37% of all body tumors. Mean age at presentation of the malignant cases was 47 years. Range of age of the small intestinal malignant cases was (3 to 75) years old. The age group that was most affected was of 41 to 60 year olds (42.2%), followed by more than 60 years old (33.3%). (66.6%) were males and (33.4%) were females, with predominance of male sex (male to female ratio 1.9:1). The exact site of malignant tumors of small intestine was duodenum (24.2%) followed by jejunum (21.2%).
Histopathological examination revealed that adenocarcinoma was the commonest tumor representing 40.5% of the tumors of the small intestine, followed by Non Hodgkin lymphoma representing 35.1%. Metastatic tumors of small intestine were 4 cases (10.8%).
Gastrointestinal stromal tumors (benign and malignant) were 2 cases (5.4%). Leiomyomas were 2 cases (5.4%). Brunner’s gland adenoma was represented only by one case (2.7%).
Concerning adenocarcinoma, the most common site was the duodenum (26%), the most common age ranged from 41-60 years (40%) and more than 60 years (40%), it was more in males with a male to female ratio (2:1) and the most common encountered grade was grade II (60%).
Concerning Non Hodgkin lymphoma, the most common site was the ileum (50%), the age group affected was more than 60 years (31%), it was more in males with a male to female ratio (1.1:1) and the most common encountered histologic subtype was diffuse large cell lymphoma (61.5%).
