Abstract

Arthrodiastasis describes a regime of articulated distraction .It acts through creating a space between the bony surfaces minimizing mechanical stress and maintaining movement , restoring the synovial circulation and encouraging fibrous repair of articular cartilage, preserving an intact and congruent femoral head. Arthrodiastasis was also used in treatment of variety of conditions such as osteoarthritis, chondrolysis and avascular necrosis as alternative to hip arthroplasty or arthrdesis in the adolescent and young adult. Arthrodiastasis was found to be effective in older children with Perthes’ disease having minimal femoral collapse(Herring grade A or grade B), so that early distraction would preserve epiphyseal height. It improved hip range of motion in all patients with late onset Perthes’ disease. Arthrodiastasis was found to be an effective method in management of Perthes’ disease in patients less than eight year (Herring grade B&C). Arthrodiastasis reduced the amount of pain and limitation in daily activities in adolescent patients with symptomatic avascular necrosis of the femoral head.
Arthrodiastatasis postponed the total hip replacement in healthy young individuals with painful stiffness or ankylosis and contractures of the hip.
Arthrodiastasis is a new approach in treatment of end stage osteoarthritis instead of joint replacement and implants having the advantage of conserving the original articulating joint.
Arthrodiastasis was used for protecting reconstruction performed for treating benign active or aggressive tumors presenting with pathologic fracture when secure fixation cannot be achieved with internal fixation implants.
Arthrodiastasis can be done by different techniques according to dieases it is used to treat and according each individual cases.
Arthrodiastasis has good results and few complications.
