Abstract Hepatic Failure is simply defined as a failure of the liver to perform its function.It can complicate almost all forms of liver disease and this will result in disturbances of different body systems especially, severe neurological manifestations due to advanced Hepa tic failure e.g.: in cirrhotic patients and usually among Egyptins of chronic bilharziasis. Although the clinical features of Hepatic Failure may differ depending upon the severity and duration of the disease, they are minimal with no encephalopathy. One of the important features is -Iaundice owing to failur of the liver cells to deal with bilirubin resulting in Hyperbilirubinaernia, but, the overall picture and lines of treatment are similar irrespective of the aetiology. Hyperbihrubinaernia in infants and children has been reported to cause sensorineural hearing loss but no such studies has been reported about the effect of Hyperbilirubinaemia in adults with Hepatic Failure on hearing. So, this study tries to show the hearing profil in patients with hepatic failure aiming to: Determine the effect of Hepatic Failure on Hearing. Correlate between duration of Hepatic Failure and Hearing loss. Correlate between Hyperbilirubinaernia and Hearing loss. Determine the effect of drugs used during history and managment of Hepatic Failure on Hearing. Afourty patients with Hepatic ”Failure that reffered to the Audiology Unit in ENT. Department by Internal Medicine Department of Zagazig University Hospitals, were used as study group and other twenty normal subjects were chosen randomly from relatives of pa.Lients and the Audiology U ni t personell as a control group. The subjects of ages, in both groups, ranged between 22-65 years and each one of them was subjected to the following: full investigations including; History, Medical exam, Otological exam, Audiological evaluation using Pure Tone Audiometer.rjbne decay and Tympanometry and Laboratory investigations for the study group only especially total serum bilirubin level. It is, there-fore, recommended that: - Patients with Hepatic Failure must be assessed audiologically, - Auditory Brain-stern Response, ABR, is needed for audiological assessment of patients with Hepatic Failure. - Drugs used in treatment of Hepatic Failure might be given orally in presence of vitamine A and B-complex with caution and close audiological follow up. - Further, Histopathological studies about the effect of bilirubin on the cochlea by means of electronmicroscopy are needed. |
