Abstract

SUMMARY
Invasion and multiplication of microorganisms into body tissue well lead to infection and illness, but for infection to occur important elements should be present (infectious agent, reservoir, host, mode of transmission) these elements called chain of infection.
Prevention of infection to occur it need to broke the chain of infection, so infection control pass through many stages of development to become Isolation Precautions which divided into two tires, the first is Standard Precautions for all patients, and apply to deal with blood; all body fluids, secretions, and excretions; non-intact skin and mucous membranes. While the second tire is Transmission-Based Precautions, this precautions have three subtypes (Air-Born Precautions; DROPlet Precautions; and Contact Precautions).
Isolation Precautions considered as a safeguard for nurses and other health care personnel. Nurses commitment or comply with Isolation Precautions is the guarantee of health for them and their families and other health care personnel. Understanding and application of all aspects of Isolation Precautions will lead to cutting the chain of infection and that is the main goal.
The present study aimed to measuring compliance of nurses with Isolation Precautions. The study carried out at fever hospitals and fever isolation units in health sector at “Assiut governorate”. The sample consisted of all nurses working in the designated sites. The total number included in the study sample was 323 at the time of collecting data.
Tools of data collection:-
1. Demographic data sheet: -
To get information about nurses’ age, marital status, qualification and years of experience. Also to get information about some of the factors which may affect the ability of nurses in compliance with Isolation Precautions, as items, reveals nurses work load (number of beds in the unit, number of nurses in the unit, and number of nurses in the unit working at each shift.
2. Observation checklists: -
The tool of the study modified and translated to Arabic by the researcher based on the previous studies and related literatures, the tool consists of two main parts, the first part is availability of resources and the second part is Isolation Precautions principles implemented by nurses for patients at different selected fever hospitals and fever isolation units.
Data collection:
Official approval to carry out this study was obtained from the different authorities. A pilot study carried out to assess tools clarity and applicability. It applied on twenty nurses from the selected hospitals and fever isolation units. Data was collected by observing availability of resources and observing each nurse’s performance three times for selected nursing procedures at different shifts. The data collection was carried out within six months started from December 2005 to May 2006. Statistical analysis was done using computer soft ware package (SPSS version 11).
The results of the present study showed that most of nurses were married, aged from 25-29, the vast majority of them had diploma of nursing school, while more than one third of nurses were had 6-10 years of experience. Nurses to beds ratio were suitable. More than half of the resources were not available for the majority of nurses. Nurses not comply with Isolation Precautions (almost all of nurses not comply with Standard Precautions, Transmission Based Precautions, steps of sample collection, hand washing steps, wearing and removing cap, mask gloves and gown, finally more than two third of nurses not comply with steps needed during and after giving I.V. solution).
In the light of the study results, the following recommendations suggested:-
1. Provide health team by knowledge about microbiology and recent biostatistics at their work place.
2. Making and provide teaching aids (posters) to enhance Isolation Precautions practices.
3. Training programs should be open to all nurses of fever hospitals according to their needs.
4. Provide a written polices and procedure for Isolation Precautions at each hospital.
5. Provide an Isolation Precautions manual for the work at each department.
6. Select two infection control nurses for each 250 beds and continually engage in training to maintain update for knowledge.
7. Follow measures that prevent infections if hospital setup not suitable for maintenance of Isolation Precautions (e.g. use alcohol for hand decontamination if sinks not available).
8. Provide suitable amounts of resources needed for work.
9. Encourage and motivate hospital and department supervisors to do further effort to maintain compliance with Isolation Precautions at work environment.
10. Maintain and apply good immunization system to highly exposed personnel.
