Abstract

The last decade has evidenced a dramatic increase in the development and utilization of pediatric HRQoL which measures the effort to improve pediatric patient health and determines the value of health care services. HRQoL has emerged as the most appropriate term for QoL health dimensions which are within the scope of health care services, thus HRQoL instrument must be multidimensional and consisting of the physical, mental, and social health dimensions delineated by the WHO (James et al., 2007). The measurement of HRQoL in pediatric oncology has been advocated for a number of years, as adulthood is now attainable for most children treated for cancer with 80% of 5-year survival rates, so the increased survival may be accompanied by long-term burden for some individuals related to the unique characteristics of their cancer diagnoses and its treatment on their educational, psychological, and social development. Thus the identification of subgroups of childhood cancer survivors at risk for poor HRQoL is important for development of new intervention strategies as well as determination of treatments promoting long-term survival and reducing risk for poor HRQoL outcomes will be a goal (Lonnie et al., 2008).
Aim of work
The objective of this study is to assess QoL of children with cancer( recently diagnosed patients) at one and three months of therapy, and to obtain effects of sex, age, type of malignancy, stage of disease and treatment on QoL of those children.
Instrument
In this study we use PCQL-32 questionnaires to measure HRQoL in pediatric cancer patient. This scale defines HRQoL in 5 items that measures impact of disease and treatment on the individual’s physical, social, psychological, cognitive functioning and disease/treatment-related symptoms. It assesses both patient’s perception and parent’s perceptions of their child’s HRQoL
Results
There is good agreement between children and proxy reports both at one and three months of therapy in that:-
• QoL of children at three months significantly better than QOL at one month of therapy in all functioning
• No significant difference between two types of malignancy and child QoL at one &three month
• No significant difference among different stages of malignancy and QoL of children at one month of therapy except for certain stages detected by post hoc test
• There is significant difference among stages of hematological, and non hematological tumors at 3 months of therapy with better QoL in early stages than advanced one
• Significant difference between chemotherapy and radiotherapy with better QoL in those children treated by radiotherapy only at 1 month.
• significant difference among three types of treatment with better QoL with chemotherapy than the others at 3 months in many domains in children report while in mother repot same result in the physical domain only
• significant difference between age groups in cognitive function, with better functioning in the youngest age group at both 1&3 months of therapy
• No significant difference between both sex in QoL both at 1 and 3 months of therapy
• No significant difference between two groups of residence in QoL both at 1&3 months of therapy
• Significant difference between two groups of SES, with better physical functioning in moderate one only at one month of therapy
• Significant difference among birth order groups in most of functioning of QoL only at one month with the worst QoL of the 4th or more order
• Significant difference between two groups of mother ages in cognitive functioning which is better in children of youngest mothers only at 1 month
• Significant difference among different groups of parent educations at 1 month as well as father education at 3month with physical& cognitive functioning of children which is better in those of highly educated parent
• Significant difference between two groups of parent occupations in some functioning of QoL which is better in children of employed parent only at 1 month
• Significant difference between two groups of family history and most of functioning of QoL only at 1month with better QoL in those of +ve family history
Conclusions
HRQoL in childhood cancer survivors mainly affected by time elapsed since diagnosis, stages of cancer and type of treatment while most of sociodemographic factors has no or little effect which is limited to 1 month only which indicates that to improve QoL of those children, we have to improve their treatment and psychological support for both children and parent to facilitate their coping with disease and its treatment.
