Abstract

According to WHO profile of TB country Egypt in 2004, the incidence of all cases per 100000 populations per year was 27, while the incidence of sputum positive cases per 100000 populations per year was 12 and the mortality was 3.2 (WHO, 2004b).
The lungs of diabetics with tuberculosis show defective defense mechanisms in the form of dystrophy of alveolar macrophages, type II alveolocytes and fibroblasts, generalized affection of pulmonary vessels, intensive fiber formation and disorganization of the pathological process (CID, 2007).
The aim of the present study is to assess the phagocytic activity of peripheral blood macrophages in patients with pulmonary tuberculosis and diabetes mellitus compared to those with pulmonary tuberculosis without diabetes mellitus to normal healthy population .
Phagocytic index and Lytic index using opsonised suspension of Candida were used as reflection of phagocytic ability in this study.
The present study was conducted upon 60 subjects, 40 patients from inpatient of Omraneia Chest Hospital, and 20 healthy subjects.
The study population was divided into 3 groups:
Group (1) : Twenty patients with pulmonary tuberculosis, with mean age (42.3 ± 4.600915) ,11 males and 9 females.
Group (2): Twenty patients with pulmonary tuberculosis with diabetes mellitus, with mean age (40.15 ± 4.094605), 10 males and 10 females.
Group (3): twenty normal healthy individuals as control group, with mean age (39.35±7.173566), 10 males and 10 females.
All patients were be submitted to the following: full History taking, plain chest x-ray, sputum examination by direct smear stained with Z.N stain for acid fast bacilli for 3 successive times, Fasting and post prandial blood glucose for diagnosis and follow up of diabetic patients, Phagocytic activity study results were expressed in terms of phagocytic and lytic indices using prepared opsonised suspension of Candida, of their peripheral blood macrophages.
from the results it is concluded that :
* Phagocytic function of peripheral blood macrophages (in form of phagocytic and lytic indices) indicated obvious phagocytic dysfunction among tuberculous patients which becomes more obvious among tuberclous patients with diabetes mellitus.
* In conclusion, this pilot study revealed that there is actually a phagocytic dysfunction As reflected by phagocytic and lytic indices among tuberclous patients and these indices inversely correlates to the concomitant presence of DM in these patients, which draws the attention to peripheral blood cell population in TB which had not taken its share of proper assessment. Hence, it would be recommended to include the peripheral blood cell population and macrophages in particular while searching in pathophysiology and management of TB.
