Abstract

This study was carried out at the internal medicine department of Ain Shams University Hospital.
In the period from 2002-2006.
This thesis aimed to detect the prevalence of paraumbilical vein patency and its possible correlation with the clinical grading in patients with hepatic encephalopathy. this thesis included 100 pateints ; they were divided into two groups:
-Group (A):Consists of 50 pateints with hepatic encephalopathy (38 males and 12 females).
-Group (B):Consists of 50 pateints without hepatic encephalopathy (34 males, 16 females).
All were submitted to thorough history taking, clinical examination and routine investigations for all subjects.
the grading of hepatic encephalopathy among group(A) were assessed, patency of PUV vein, portal vein velocity, C.I., presence of ascites and upper endoscopic examination.
The statistical analysis of the results of this thesis showed that patients with hepatic encephalopathy (group A) had significantly higher prevalence of patent PUV than patients without hepatic encephalopathy (group B).
Patients with higher grades of hepatic encephalo-pathy showed higher prevalence of PUV patency with significant difference.
Ascites was significantly lower among group (A) than group (B).
Portal vein velocity was higher among group (A) than group (B) with no significant difference.
Patiets with patent PUV had higher prevalence to have low grade esophageal varices with statistically significant correlation between PUV.
Conclusion:
The conclusion of this work is that:
1-Pateints with patent PUV had higher incidence of hepatic encephalopathy.
2-There was correlation between PUV patency and clinical grading of hepatic encephalopathy.
3-A significantly enlarged PUV protect against formation of large varices and thus less liekly variceal bleeding.
