Abstract

The thalassemia syndromes constitute the commonest of all gene disorders
the world. ~- Thalassemias in particular are common in Mediterranean countries
, Egyp~ and because frequent blood transfusion is mandatory in managing such
, ts, they are prone to blood borne infections especially Hev which is prevalent
Prognosis is quite different among individuals infected with HeV; 80% of HeV
cases will develop chronic hepatitis and about 20 to 50% of these chronic cases
develop cirrhosis after 10-20 years or longer. HeV-RNA positivity (Hev
’a) can be considered a sensitive marker for significant liver damage and chronic
disease and the true healthy carrier state is rare.
In HeV infection, both the susceptibility and the course of disease may depend
!he difference in the immune response which in turn may be affected by HLA status,
rtpJays a crucial role in antigen presentation.
The human HLA complex occupies a segment of somewhat less than 4
, organs on the short arm of chromosome 6. It can be subdivided into 3 regions
eeting the HLA class to which the gene products belong. Genes encoding class I
ducts are telomeric; those encoding class II products are centromeric and those
ding class ill products occupy an intermediate position between these two regions.
