Abstract

There are number of strategies that may be adopated for evaluating the peritoneal, mesenteric and omental pathologies. Plain X-ray studies and and abdominal Ultrasound were used to evaluate the possible lesions that may be localized to the peritoneal cavity and membrane, however being limited in evaluating the spread and origin of the lesions.Before CT based techniques had been stabled; laparoscopy and even surgical maneuvers were used to evaluate the possible peritoneal spread of a known peritoneum favoring tumor.CT based techniques allow the potential to visualize the all spaces of the peritoneal cavity, and to assess the spread of possible peritoneal disease and also to determine the abdominal origin of metastatic peritoneal deposits.The study included 100 patients having different peritoneal pathlogies. They were 40 males and 60 females.Review of the clinical history of the pateint as well as the presenting signs and symptoms with CT examination was done searching for peritoneal pathology.Development of the coelomic cavity and the mesenteries was described with illustrative diagrams. The normal CT anatomy with demonstration of the various abdominal and pelvic peritoneal reflections,folds and ligaments are also described.Different pathologic problems were reviewed as regards to their CT appearance with emphasis on picture and special predilection of abscess formation within the different abdominal and pelvic locations that was described with demonstration by illustrative CT pictures. Neoplastic conditions including the benign and malignant tumors; primary as peritoneal mesothelioma and secondaries; were described.The results of the CT scan of the abdomen & pelvis of the 100patients revealed malignant peritoneal pathology in 56 patients,inflammatory peritoneal pathology in 32 patients and miscellaneous peritoneal lesions in 12 patients.The results revealed that inflammatory peritoneal pathologies are common; with common abscess formation specially in the subphrenic space, and in pelvic spaces. The secondary peritoneal malignancies are much more common than the primary and the ovarian& colonic carcinomas had the upper hand.The all results were correlated to pathologic and/or surgical findings.All results obtained were tabulated, discussed as regards to their clinical background, CT and pathologic findings.In conclusion we found CT of much more benefit in searching for suspected peritoneal patholgy; not only for detection,characterization and staging; but also for guiding biospy for tissue and fluid sampling.
