Abstract

The present study included 20 patients in one group, of different sites of head and neck cancer treated by 3-D conformal radiation therapy technique at Kasr El-Ainy Centre of Clinical Oncology and Nuclear Medicine (NEMROCK) and clinical oncology center of Zagazig University. The aim of this study was to evaluate the importance of new technology evolved in radiation therapy (3D-CRT) and to show its value in decreasing treatment related toxicity. Our group treated with conventional fractionated schedule, 1.8Gy/fraction, once per day, a total dose of 70Gy in a total duration of 8 to 9 weeks with delivery of high dose per fraction to the gross tumour volume (GTV), and the planning target volume (PTV) while giving lower dose per fraction to the organs at risk (ORS).
Patients were assessed for treatment related toxicity using the European Organization for Research and Treatment of Cancer and the Radiation Therapy Oncology Group, (EORTC/RTOG).Patients follow-up period range between 4 and 32 months, toxicity was divided into acute toxicity during radiation therapy and late toxicity that occurred at least 90 days post treatment.
The median age of our group was 51 years
The female to male ratio was 1:4,
Staging was corrected according to the AJCC 2006 staging system, there were more T3 (75%) than T4 (25%) in patients, also the nodal status was more advanced.
The patient’s compliance for our group was 95%, while one patient (5%) his radiation therapy was interrupted and the tolerance to treatment was worse.
In general acute toxicity was tolerable and complete healing was the rule.
Skin, mucous membrane, and subcutaneous tissue toxicity were mild and with no significant problems for the patients.One patient complained of L’Hermitte’s syndrome which is a momentary electric like shock that is triggered by flexion of the cervical spine, condition started 3 to 5 months post radiation therapy and was self limited with no sequale.
All patients entered in complete remission (CR) after the initial treatment, local recurrence occurred in 2 patients and distant metastasis in 2 patients.
