Abstract

The evaluation of right ventricular function is difficult due to its geometrically complex shape with its asymmetric crescent cut section and trabeculated walls. Our study aimed to evaluate right ventricular function by echocardiography in patients with pulmonary hypertension and determine the relationship between parameters of RV function and BNP levels. The study included 90 patients with pulmonary artery pressure greater than 40 mm Hg together with 30 normal volunteers as control group. Both groups were subjected to standard and pulsed tissue Doppler echocardiography to estimate. Pulmonary artery pressure from tricuspid regurge by Bernoulli equation.RVEF by Simpson method from 2-D 4 chamber view. Tricuspid annular plane systolic excursion from M-mode 4 chamber view. RV Tei index from pulsed tissue Doppler of tricuspid valve. S wave by pulsed tissue Doppler at lateral tricuspid annulus. Then blood sample taken immediately to measure plasma BNP level.using ELISA technique The study showed a significant negative correlation between pulmonary artery systolic pressure and right ventricular parameters (RVEF, TAPSE, and S wave), and showed a positive correlation between pulmonary artery systolic pressure and right ventricular Tei index. It also showed that RV TEI index, tricuspid annular plan systolic excursion and peak systolic velocity(S wave) are well correlate with BNP plasma level.
