Abstract

The primary immunodeficiency disorders (PID)are a heterogeneous group of genetically determined disorders. Increased frequency of lymphoproliferative diseases (LPD) is a well recognized complication with variable risk. Increased risk of malignancy in patients with primary antibody deficiency has been reported. So we sought to evaluate the humoral immune status of children with lymphoid malignancies in order to consider the possibility of primary immunodeficiency as a risk factor of malignancy through a cross sectional study which included 62 patients who were diagnosed as lymphoid malignancy (Hodgkin’s lymphoma(HL), non Hodgkin’s lymphoma (NHL)or acute lymphoblastic leukemia (ALL)).These patients were systematically recruited from Ain Shams University,Pediatric Hospital, Department of Hematology and Oncology and from the National cancer Institute, pediatric oncology clinic in the period from June 2008 to February 2009. The patients were subjected to:
1) Full history taking focusing on history of repeated infections.
2) Full clinical examination including manifestations of malignancy and manifestations of repeated infections.
3) Immunodeficiency related (IDR) score was used to verify severity and number of patients with IDR score ≥ 8 and ≥ 6 was calculated. An IDR score of 6 or more is considered as the threshold indicating a significant number of illnesses, while an IDR score of 8 or greater is supposed to have positive predictive value of having immunodeficiency.
4) Laboratory evaluation was done including a complete blood count with differential and measurement of Serum immunoglobulins (Ig)(IgA, IgM, and IgG) by nephelometry in 32 patients before start of chemotherapy.
