Abstract

This is a descriptive study of 375 elderly patients known to be chronically diseased with (hypertension , diabetes , chronic liver disease and COPD) . attending out patient clinic of family medicine, Suez canal university hospital.
The results of the study inform that patients are distributed into male 53.6% and female 46.4% that correlate with sex ratio of total population in Egypt of 1.02 male(s)/female .
The young old patients(65-74 years) are more representative in this study 94.4%than old old patients(75years and above) 5.6%.
The percentage of elderly hypertensive patients is 33.3%, elderly diabetic patients is 25.6%,chronic liver disease 10.4%.
The prescribing pattern to elderly hypertensive patients in this study represents that angitensin converting enzyme inhibitor is the most prescribed drug in combination to amlodipine 47.7% , ACE& thiazide, propranolol, 22% which mean that the prescriber s are following guidelines.
This study show that oral hypoglycemic drugs especially sulphonyl urea drugs are most frequently prescribed drugs then biguanide was the second prescribed drugs as mono therapy or in combination.
This high frequency of diabetic elderly women should direct us to more control of other risk factor as diabetic women have worse control of their blood pressure, blood sugar and are more susceptible to heart disease.
the percentage of elderly patients who receive in- appropriate prescribing is 38.3% according to the Beers list of in appropriate drugs.
The percentage of the low severity drug (indomethacin) represent 17.1% of the all in appropriate drugs prescribed to elderly patients in this study which represent the high percentage followed by propranolol 10.4%and GIT antispasmodic 10.1%, Diazepam is the least prescribed to elderly in this study 0.3%
It is estimated that about 70% of the elderly patient are adherent to their medications and about 28.9% take their medications correctly 100%.
It is obvious that in this study that age of the patient, total number of prescribed drugs , number of chronic diseases affect elderly patient, and fear of side effects all are significant factors affect the adherence and intake of elderly patient to his medications.
