Abstract Aim of the work To determine the incidence of perinuclear anti neutrophil cytoplasmic antibody (P-ANCA) in a group of patients with rheumatoid arthritis and whether they have any clinical correlation. The results of the study: Perinuclear antineutrophil cytoplasmic antibodies (P-ANCA) were detected in 16 patients of RA group (53.3%) and in one case of the control group (6.6%). There was a highly significant increase in the incidence of (P-ANCA) in the RA group when was compared to the control group (P<0.006). Rheumatoid arthritis patients were further divided into (P-ANCA) negative group and (P-ANCA) positive group, when both groups were compared together regarding clinical and laboratory variables, no statistical significant difference was observed, a part from the statistically significant higher ESR in (P-ANCA) positive group (P<0.007). No significantly correlation was observed between (P-ANCA) titer and any of the studied parameters, except for a significant positive correlation between titer of (P-ANCA) and ESR (P<0.03). Accordingly, since ESR is not specific and that many other factors influence it, in view of our results, it seems that rheumatoid arthritis patients with (P-ANCA) positive tests do not represent a specific subgroup of rheumatoid arthritis and are not at risk for specific complications. The specificity of (P-ANCA) test to rheumatoid arthritis patients was 93%. The sensitivity of (P-ANCA) test to rheumatoid arthritis patients was 53%. |
