Abstract

The most important responsibility of the anesthesiologist is“management of the patient’s airway”.
Fiberoptic intubation is useful in managing patients with difficult airways.
The technique is easier in elective cases, but can be used by skilled practitioners even in cases of unrecognized difficult airway.
Fiberoptic intubation can be used in awake/sedated patients,asleep/breathing patients and asleep/paralyzed patients, but its usage in spontaneously breathing patient is the gold standard and technique of choice for the elective management of a difficult airway.
The aim of this study was to compare awake fiberoptic intubation using local anaesthesia with conscious sedation versus general anaesthesia using sevoflurane in spontaneously breathing adult patients with difficult airway.
Patients were divided into two equal groups each group contains 20 patients: (group G) fiberoptic intubation after induction of general anaesthesia using sevoflurane, (group L) for awake fiberoptic intubation using local anaesthesia to block the airway reflexes.
In the present study, we assessed the safety of both techniques on patients with difficult airways by their effect on haemodynamics as systolic,diastolic blood pressures, heart rate, oxygen saturation, and we measured the incidence of complications during the fiberoptic intubation as hypertension,tachycardia, dysrhythmias, bleeding due to soft tissue trauma, hypoxia,laryngeal spasm, sore throat and patient satisfaction.
