Abstract Aim of the work Is to determine and compare the effects of continuous oral administration of propranolol on the portal venous pressure in patients with cirrhosis with those effects of transdermal nitroglycerine on the portal venous pressure in the same type of patients. As well as, compared the effects of the combination of both drugs. Also, it was aimed to assess hepatic function, sonographic findings of portal venous system, endoscopic changes of esophageal varices, and the changes of portal venous pressure using percutenous trans-splenic portal manometry before and after administration of these drugs for six months. Conclusion Transdermal nitroglycerine in 10mg dosage may be a beneficial new approach in the medical management of portal hypertension which is associated with a significant improvement in liver function tests. While combination of both propranolol and Transdermal nitroglycerine resulted in better results than each one alone. The use of sonographic measurements of portal vein diameter, splenic vein diameter, splenic size. Which is an easy, non invasive, available maneuver may be of beneficial value in early diagnosis, follow up and management of portal hypertension and esophageal varices secondary to chronic liver disease. |
