Abstract Aim of the work To provide a brief review of transpupillary thermotherapy (TTT) technique, application, indications, advantages, complications and comparison between other modalities of treatment. Summary Advantages of TTT for treatment of tumors include more rapid visible reduction of tumor over a period of a few months, more focal treatment with less destruction of surrounding normal tissues, and treatment as an outpatient procedure not requiring hospitalization and reducing over all patient costs, and the heat induced damage to the ocular structures may not be clinically evident for months or years. An important advantage of combined plaque radiotherapy is the low rate of recurrence compared to primary TTT. The disadvantages of TTT for tumors include multiple treatment sessions, immediate destruction of overlying retina, and inability to treat large tumors or those situated in the peripheral fundus. The near infrared wavelength is absorbed by melanin contained into retinal pigment epithelium (RPE) cells and choroidal melanocytes. The aim of the treatment of TTT is to produce an interlessional temperature rise of 4-9 °C, through the exact mechanism of action is still unclear. The probable one is vascular thrombosis precipitated by cytotoxic free radicles released from irradiant tissue. Then induce leakage reduction and exudates resorption. In this event fovea flattens and potentially vision stabilizes or improves. Relatively rapid involution of CNV following TTT, prevents subretinal scarring and permenant photoreceptor damage, therapy prevent central and permenant scotoma in patients with subfoveal CNV. |
