Renal stones are one of the oldest entities known to humankind. The first evidence of urinary stones founded in an Egyptian mummy at E1 Amrah - Egypt (4800 B.C). Kidney stones are one of the most common disorders of the urinary tract. Each year, people make almost 3 million visits to health care providers and more than half a million people go to emergency rooms for kidney stone problems. It always presumed that ESWL would increase the risk of calculus recurrence. The cause thought to be persistent small calculus fragments after ESWL. Using follow-up ESWL does not increase the incidence of recurrent calculi. Recurrence may be higher if ESWL attempted for larger calculi (Kosar et al., 1999). The patient study aimed to identify effect of combining herbal therapy (Ammi visnaga) with extracorporeal shockwave lithotripsy on the stone clearance rate for patients with renal stone. Research Design: - The study utilized the quasi-experimental design. The quasi-experimental study used to identify the effect of combining herbal therapy (Ammi visnaga) with extracorporeal shockwave lithotripsy on the stone-clearance rate for patients with renal stone. Research Setting: The study was conducted at ESWL unit of Urology Department, Menoufyia University Hospital, Egypt. This setting was thought to be representative for patients with renal and ureteral stones. Data collection was extended from the period of June 2006 to the middle of March 2007 Subjects: The study subjects comprised of 50 adult patients with the diagnosis of renal calculi were selected and divided alternatively into 2 equal groups, Study group (I) was received extracorporeal shockwave lithotripsy and maintain taking daily-prescribed dose of Ammi visnaga and follow therapeutic regimen relating to (drugs, diet, exercise, sleeping position, fluid intake). Control group (II) was received the conventional therapy only (extracorporeal shockwave lithotripsy, and follow therapeutic regimen relating to (drugs, diet, exercise, sleeping position, fluid intake). Patients of both groups were matched as much as possible as regards to: 1. Conscious patients. 2. Adult of both sexes. 3. Single or multiple stone. 4. Stone burden is 2 Centimeter or less. 5. Normal vital signs. 6. Not using Anticoagulant or Ca channel blockers. 7. Free from any other associated diseases such as kidney disease, liver disease, heart disease, diabetes, ect. Variables: The independent variable is using extracorporeal shockwave lithotripsy and ammi Visnaga while the dependent variable is the stone clearance rate after ESWL. Tools of the study: To achieve the aim of the study and to collect the necessary data, the investigator used four tools based on review of the related literature. All tools were translated into Arabic. The following tools were used: Tool I: Knowledge assessment (structured interview questionnaire). Tool II: Urological assessment sheet. Tool III: Nutritional assessment sheet. Tool IV: Evaluation sheet. Tool I: Knowledge assessment: structured interview questionnaire (Appendix II): It was developed and used by the researcher to assess patient’s knowledge about disease process and its management, after reviewing the related literature. It was in Arabic and comprised of three parts as the following:- I) Part one: Sociodemographic data It included data about age, sex, residence, level of education, employment, marital status. II) Part two: Medical history It included data such as past, present, and family medical history, previous hospital admission for ESWL and date of discovery of disease.
