Abstract

Summary and conclusion
The current study is a prospective study was carrying out in Minia oncology center during the period from June 2005 to march 2006 , the main objective of this study was to compare the palliative effects of a single fraction of 800 cGy radiotherapy and the multiple fractions of 3000cGy / 10 fractions radiotherapy on painful bone metastases in a period of follow up of two weeks post treatment .
External-beam radiotherapy has become a mainstay in the palliative treatment of metastatic bone disease. The indication for radiotherapy in metastatic bone disease is the relief of pain.(20)
The study was involving sixty patients of both sex with painful bone metastases of any primary solid tumor excluding malignant melanoma, our patients were randomly assigned to two groups: group A, including thirty patients, received 800 cGy single shot. And group B, including thirty patients , received 3000 cGy divided on 10 daily fractions .
Our patients were only treated by radiotherapy during the period of follow up (2 weeks) to exclude any effect of other modality of treatment.
Our patients were treated using linear accelerator machine ( Philips SLI 15) with energy 6 to 10 MV after being simulated.

Evaluation of our patients before treatment was fulfilling the following
• Full history and full clinical examination.
• Radiographic evaluation of the treated site including plain x ray and bone scan.
• Laboratory investigations including complete blood count (CBC), liver function tests (LFT), kidney function tests (KFT), and serum calcium level.
During and post treatment evaluation was involving the previously mentioned questionnaire (table 2). In addition to weekly CBC and serum calcium level.
In the current study, we found that there was no significant difference between both groups , and the patients in both groups were markedly improved and the improvement in both groups occurred to the same extent with no complication occurred to all patients in both groups.
In the present study, recurrence rate was higher in group A (single dose) than in group B, but with insignificant difference. The recurrence of pain started after 4 weeks in both groups.
The current study revealed that re-irradiation (re-treatment) was carried in 2 cases (6.6%) in group A (800 CGy single shot), with no reported re-irradiated cases in group B (300 CGy x 10). There was insignificant difference regarding this finding.
