Abstract

Trauma has become a public health pioblem all over the world, and ascular trauma is an clement of this problem.The surgeon is commonly Faced with ethical and clinical dilemma in ease if extremity vascular trauma as he should decide to manage either by amputation ir salvage Multiple assessment scales have been developed to assist the surgeon m he limb management decision-making process. The present work aimed to evaluate the role of scoring systems and radiological examination in assessing the severity as well as predicting the outcome of vascular injuries of the extremities in patients admitted to Emergency Department in Alexandria Main University Hospital within si* months duration. The study entailed 136 patients with liauma to the estremilics who were admitted to Kmer^ency Department from the first of May till the end of October 2007 All patients were subjected to through history taking, clinical examination, radiological, heamato log teal and biochemical investigations and finally three scoring systems were applied and used for assessment .Tbe results revealed that: The a£c of patients ranged from 4 np to 63 years with a mean age of 31 82 ± 12.49 years.Males outnumbered females (S3 R% and 16.2% respectively) with a sex ratio of 5.2:1. The cuncnt study showed thai the most common cause of vascular trauma to the extremities was Road 1 raffle Accidents (RTA) (84 5%) tut lowed by cut wrist which constituted 12 5% of patients. Falling either from a height or downstairs were 10 3% and 8,8% respectively Firearm injuries constituted 7 4%. Injuries caused by a machine or stab injuries were of the same incidence 5.1% each The least percentage of injuries was iatrogenic injuries 2.2%. (t was noticed that lower limb injuries {69 1%) were more common than upper limb injuries (30 9%) amony the studied patients. As regaids the site of arterial injury, it was found lhat the most common artery to be injured was the femoral arter> (27.9%) followed by popliteal and tibial arteries {20 6% each) .
