Abstract

In this retrospective study, the files of 1170 patients with metastatic breast cancer who presented to ACOD fom Jan. 1994 to Dec. 2003, were reviewed to study clicopathological features and t evaluate treatment results.
In the current study, the median age of patients was 49 years, only 11 patients (0.9%) were males, 51.3% were premenopousal and 48.7% were postmenopousal, 70% had ≥3 children, and 14.4% had family history of breast cancer.
Left sided breast cancer was slightly more than the right sided breast cancer, and U.O.Q. lesions was the most common site.
Modified radical mastectomy was the most common type of surgery performed in our patients, 90% of patients had IDC, 98.6% had high grade and 53.7% had hormone receptor positive tumors.
Most of patients had T2 or higher and had positive axillary LNs.
In this study, 869 patients received adjuvant treatment with 2/3 of patients developed recurrence withen 2 years ( 28.4% had locoregional recurrence while 71.6% had distant metastasis).
Bone followed by liver and lung were the most common site of metastasis in our patients.
In this study, 792 patients received chemotherapy (CAF, Navelbine + 5-FU, CMF, Navelbine + Adriamycin) while 378 patients received hormonal therapy (AIs, Tamoxifen, Megace, Ovarian ablation).
The ORR of our patients was 51.8% with 11.8% achieved CR.
The response rate was higher in patients who had DFI > 2 years, HR +ve, solitary site of metastasis and bone or soft tissue metastasis.
ORR of CAF, Navelbine + 5-FU, Navelbine + Adriamycin was significantly higher compared to that of CMF, while there was no significant difference between different hormonal agents.
In this study, there was no significant difference in hematological toxicity of different chemotherapy regimens but regarding non hematological toxicities, we found that patients who received Navelbine +5-FU had a significant lower incidence of grade 1&2 alopecia compared to other regimens, while vomiting was common in patients who received CAF and CMF.

All types of hormonal agents were tolerated by patients with acceptable toxicity.
Hot flashes and vaginal discharge were significant common in patients who received Tamoxifen, while wight gain was significantly higher in patients who received Megace.
The median survival of our patients was 12 months, The one year survival was better in patients who achieved CR, in patients who had DFI > 2 years, HR +ve, solitary site of metastasis and bone or soft tissue metastasis.
