Abstract

Laryngotracheal stimulation during laryngoscopy and trachea! intubation can give rise to haemodynamic responses in the form of tachycardia, arterial hypertension, arrhythmias and pulmonary arterial hypertension. These responses may be more obvioius during prolonged direct laryngoscopy.
Many methods are sued to overcome the cardiovascular stress response associated with laryngoscopy and endotracheal intubation. Magnesium sulphate may be a very useful method of controlling the hypertensive response as it nol only inhibits the rise in arterial blood pressure but also prevents catecholamine release from the adrenergic nerve terminals and from the adrenal glands and exerts a significant anti-arrhythmic effect.
The aim of this work was to evaluate the efficacy of different doses and methods of administration of magnesium sulphate in attenuating the pressor response during prolonged direct laryngoscopy. This work was carried out in otorliinolaryngology department in the Main Alexandria University Hospital. Sixty adult patients (ASA class I & II) schedualcd for prolonged direct laryngoscopy were studied. Those patients were selected in the age group between forty and sixty years old. They were randomly allocated into one of four groups (fifteen patients each).
