Abstract

Hepatocellular carcinoma (HCC) is a lethal malignancy that is considered one of the most common malignancies in Asian and African population with a median survival ranged between 1.6 -8.3 months for untreated patients. HCC affects males more than females in a ratio of 2-4:1 It is more prevalent in persons about the age of 45years specially those with chronic active hepatitis B or C virus infection and cirrhotics.
The only curative treatment for HCC is surgery but HCC patients usually present in late advanced inoperable stage.
There is no single or combination chemotherapy had proven effectiveness against advanced HCC and non gave a response rate more than 25% and similarly was the results of immunotherapies.
Local ablative therapies like PET and TACE had shown promising results especially in early small HCCs.
This work was done to evaluate the role of PEI versus local radiotherapy in the treatment of HCC patients.
Forty patients were assigned into two groups, twenty patients each, group A received U/S guided percutaneous ethanol injection into the HCC lesions using 95% sterile alcohol in multiple sessions according to the size and number of lesions.
Group B received 21Gy (300c Gy * 7 fractions) external beam
radiotherapy using two or tliree portals with or without wedges according to the size and location of the lesions using body contours to obtain.
