Abstract The aim of this study is to evaluate the role of computed tomography in assessment of patients with chronic sinusitis prior to endoscopic sinus surgery. This study revealed that the obstructive inflammatory disease was the most common inflammatory disease that found to affect the Sino-nasal cavity and responsible for chronic sinusitis. Three obstructive patterns were detected in this study (OMU, infundibular and SER patterns). Many anatomic variations were detected affecting the region of the ostiomeatal complex and contribute either individually or in combination with each other in the pathogenesis of the obstructive inflammatory disease. These anatomic variations were the most common predisposing factors detected in this study to be responsible for occurrence of OMU and infundibular obstructive inflammatory patterns. Sporadic findings were found affecting a large number of the examined patients in this study either individually or in combination with other groups. Sinonasal polyps were the least detected group. Analysis of the CT findings should be undertaken in a routine, standardized fashion to insure that important details are not misses for more accurate evaluation of sinus disease and complete covering of the region of the ostiomeatal complex. To aid in this evaluation, a CT reporting scheme was developed and focuses on the most important structures which should be covered on routine examination of the paranasal sinuses. Regular preoperative analysis using this system allows the recognition of hazardous anatomic variations and pathology which increase the safety and efficacy of paranasal endoscopic sinus surgery. |
