Abstract

We aimed in this study to assess the changes in mesenteric and renal blood flow in hyperbilirubinemic neonates under phototherapy and to correlate these effects with different factors that may affect the mesenteric and renal blood hemodynamics.
The study included 30 preterm cases with exaggerated physiological unconjugated hyperbilirubinaemia, and 20 apparently doing well preterm neonates as controls.
All infants included were subjected to:-
History taking, thorough clinical examination, laboratory investigations were done (complete blood count, serum bilirubin level, blood group, coomb’s test, reticulocytic count, serum electrolytes whenever indicated ,c. reactive protein, and arterial blood gases) in addition to mesenteric and renal duplex- pulsed Doppler ultrasound to determine the blood flow velocity in the superior mesenteric and renal arteries.
The present study demonstrated an increase in the resistive index of blood flow in the superior mesenteric artery after starting phototherapy in comparison to before it, which didn’t return to the pre phototherapy values after its discontinuation.
Regarding the peak systolic velocity of the superior mesenteric artery in the present study, there was a highly significant decrease during phototherapy in comparison to before it, and then it almost returned to pre-phototherapy values after withdrawal of phototherapy.
