Perioperative myocardial injury is one of the most important predictors of short- and long-term morbidity and mortality associated with non-cardiac surgery.
Prevention of perioperative myocardial injury is thus a corner stone for an improvement in overall postoperative outcome.
Activation of myocardial sympathetic nerves can result in myocardial ischemia especially in patients with coronary artery disease. Consequently, a reversible cardiac sympathetomy by high thoracic epidural anesthesia (TEA) may have anti-ischemic effects owing to the blockade of efferent sympathetic fibers.
