Abstract

The role of H. pylori infection in the pathogenesis of duodenal ulcer is well known. However, the correlation between perforated ulcer and H. pylori infection shows a great debate in literature.
The relation of H. pylori and duodenal ulceration was proved, and many mechanisms by which the pathogenesis of ulceration occurs were discovered. These mechanisms include many virulent factors whether morphological like the flagella, or through the secretion of toxins.
However, the relation between H. pylori and perforated duodenal ulcer remained a major point of argument. The results in literature were divided into three groups.
The first group suggested a positive relation with increase in the incidence of perforation with the higher rate of infection.
The second group claimed that the presence of H. pylori did not increase the incidence of perforation.
The third group suggested that the presence of H. pylori my provide protection against perforation.
In this work, we studied 60 patients, 30 patients of them presented to the emergency department in Cairo University hospitals with perforated duodenal ulcer and 30 patients presented to the gastroenterology department with non complicate duodenal ulcer.
